When the muscles in your buttock and low back are part of what is irritating the nerve.
Sciatic-type pain has several possible sources, and massage helps with some of them and not others. Massage Therapy for Sciatica helps clarify which is the useful part.
Massage helps most where deep gluteal muscles are irritating the sciatic nerve as it passes through the buttock, or where low back muscle guarding is contributing to the problem. The piriformis and surrounding deep rotators sit directly over the nerve, and when they are tight and irritable they can reproduce a convincing sciatic pattern.
Massage helps less where the nerve is genuinely compressed at the spine by a disc or by narrowing of the canal. It can still ease the secondary muscle guarding, which is worth something, but it is not treating the compression.
That is why assessment comes first. Physiotherapy leads where there is true nerve involvement; massage leads where the driver is muscular.

Piriformis and the deep external rotators, where the nerve passes and where tightness most often refers down the leg.
The low back muscles that guard once sciatic symptoms start, and then perpetuate them.

Frequently tight in a leg that has been guarded for weeks, and part of why walking still feels wrong.
Some sciatic presentations are medical rather than muscular. Seek medical care the same day for numbness in the saddle area, loss of bladder or bowel control, or progressive weakness in the leg or foot. These can indicate cauda equina syndrome and are a surgical emergency.
We screen for these before treating, and we will send you on rather than put you on the table.
